SYNSORMED ANNOUNCES PARTNERSHIP WITH HI-BRIDGE SOLUTIONS

SynsorMed, a leader in Remote Patient Monitoring and Remote Therapeutic Monitoring healthcare, is pleased to announce that they have partnered with HI-BRIDGE Solutions on a Georgia Department of Health Grant to provide RPM services to providers in Georgia.

Atlanta, GA, August 16, 2022. SynsorMed today announced that they’ve recently partnered with HI-BRIDGE Solutions on a Georgia Department of Health Grant to provide RPM services to providers in Georgia.

HI-BRIDGE Solutions is the flagship product of the Health Information Division within the National Center for Primary Care (NCPC) at Morehouse School of Medicine. The division provides education and technical assistance services that support primary care stakeholders throughout Georgia, the region, and the nation with the health IT and interoperability priorities of the Center for Medicaid and Medicare services and the Office of the National Coordinator for Health Information Technology (ONC).

HI-BRIDGE Solutions, formerly GA-HITEC, served as Georgia’s Regional Extension Center from 2009 – 2016 through a cooperative agreement with the ONC’s program to transition primary care physicians from paper to electronic health record technology solutions. This important work continues through additional education and training opportunities that support clinicians in advancing patient care through technology.

This contract aims to promote the adoption and use of Electronic Health Records (EHR) and Health Information Technology (HIT) to improve provider outcomes and patient health outcomes related to identifying individuals with undiagnosed hypertension and managing adults with hypertension. The Contractor has supported practice transformation through electronic health record (EHR) technology and a broader scope of health information technology (HIT) since 2010.

The Contractor has partnered with community and state organizations to assist clinicians in meeting the regulatory requirements for meaningful use. Through this work, ambulatory practices and public health departments have successfully integrated technology into their clinical works to provide enhanced patient care.

Carmen Hughes, the Health IT Division Director, NCPC, expressed, “HI Tech is proud to partner with SynsorMed in delivering home patient monitoring to patients in need. This program will enhance patient care for patients in hard-to-access areas across Georgia.”

HI-BRIDGE Solutions funding supports the practices that implement electronic tools, generate customized reports for population health analytics, maximize utilization of their EHR functionality, and increase focus on managing hypertension.

“According to recent studies, cardiovascular deaths related to high blood pressure, which many call a silent killer, continued to rise over the last two decades, which also showed unambiguous health inequities. In those studies, African Americans had a nearly twofold higher mortality rate than their white peers for hypertension-related heart disease deaths.,” stated Theo Harvey, CEO, and Co-Founder of SynsorMed.

The data generated by remote monitoring and its impact on the ability of the clinician to proactively respond to feedback in real-time align perfectly with the goals of utilizing various tools to manage hypertension
